I grant APX Performance Photography and its legal representatives the irrevocable right to photograph my child during sporting events, practices, or related activities. I authorize the use of these photographs for: Website and portfolio display Social media promotion Marketing and promotional materials Editorial or informational use related to sports photography I understand that no compensation is provided and that all photographs remain the property of APX Performance Photography. This release does not authorize use in defamatory, misleading, or inappropriate contexts.

I certify that I am the parent or legal guardian of the minor named above and have authority to grant release.

This form is provided for parents or guardians when a model release is required for youth sports photography.
